Bilingual Case Manager

Essential Job Responsibilities:

  • Facilitates or assists in facilitation of the strategic planning process for case management services to clients and their families
  • Engages families, stakeholder, informal support and other involved in the clients lives for the primary purpose of developing strengths-based plan, which meets the family’s needs
  • Responsible for CFT process related documents including strengths, needs and culture discoveries, individualized service plans and support plans
  • Works collaboratively with Care Manager Supervisor
  • Facilitates or assists in facilitation of CFT meetings
  • Provides information, education, and supports to clients
  • Develops, implements and reviews as necessary, a relevant crisis and safety plan for the family
  • Works with patients and families to achieve independence/self-efficacy
  • Assists patients in securing needed services
  • Coordinates all treatment instructions from the Clinical Director
  • Facilitates treatment team meetings
  • Assists the Clinical Director in program development and continuous quality improvement
  • Demonstrates appropriate judgment skills to be able to make independent clinical decisions in routine patient care matters
  • Evaluates / assists in evaluation of appropriateness of Plan of Care and revise on an ongoing basis with communication to all other disciplines
  • Acts as a comprehensive care coordinator for patients assigned to his / her care, assessing the patients’€™ needs accordingly and facilitate communication amongst other clinicians providing care.
  • Performs comprehensive assessments of physical, emotional, psychosocial, and environmental needs of the patient including outcome and assessment information.
  • Completes and submits all required documentation within established company guidelines.
  • Participates in development of patient goals and discharge plans as well as makes revisions to these based on the changes in patient status

  • Communicates in a positive, respectful, and courteous manner.
  • Practices active listening skills and uses partnering communication strategies
  • Interacts with coworkers and the community to build productive and collaborative relationships to support the achievement of agency and program goals.
  • Understands and maintains private and sensitive information within the confines of need-to-know parameters.
  • Reviews email daily using various writing styles and formats to produce effective communication including a clear purpose and intent.
  • Maintains a calendar on a weekly basis that is available to supervisors and coworkers

Functions as an effective team member with co-workers and the community to build productive, collaborative relationships that support achievement of program goals and objectives.

  • Shares pertinent information with director, contractors, and team members regarding work progress, successes, barriers, concerns, and other areas that may have an impact on the department’s services or image, may affect relationships with key participating parties within the confines of need-to-know.
  • Attends and participates in staff meetings in accordance with department standards and schedule. Supports the decisions of the director and members of the management team to staff, participants, and the community.

Utilizes supervision effectively, takes initiative to secure professional development opportunities, and applies relevant information to the work setting.

  • Prepares and participates in supervision that is regular, reflective, and collaborative in nature.
  • Recognizes the personal limits of competencies and expertise, seeks supervision and guidance accordingly.
  • Maintains a reasonable level of awareness of current professional information in fields of activity and undertakes regular and ongoing efforts to maintain competence in the skills used.
  • Notifies supervisor of crisis situations, unusual incidents, and/or on-going problems within appropriate timelines (including reports to Child Protective Services).

Education, Experience, and Other Qualifications:

  • Proven experience in the provision of behavioral health services
  • 2 years of experience as a case manager
  • AA or Bachelor’€™s in human services, social work, or related field
  • Strong written and verbal communication skills
  • Detail oriented
  • Strong organizational skills
  • Bilingual in Spanish Required
