New Patients

If you have never received services by a Bayless provider, please click the button to fill out the attached request form.

Upon completion, a Bayless engagement center representative will respond via email or phone call within 24 business hours.

DISCLAIMER: If this is an emergency call 911. If you are currently experiencing a crisis please call the crisis hotline at 602-427-4600. If for any reason the patient is unable to keep the scheduled appointment it is the responsibility of the Patient, Parent, and/or Legal Guardian to call Bayless staff, at least twenty-four (24) hours in advance, to cancel. If the Patient fails to cancel their appointment at least twenty-four (24) hours in advance, a $50 no show fee will be applied. This fee must be paid prior to being seen for any Medical, Dietetic, Psychiatric and/or Counseling appointment.  Note: All AHCCCS patients are exempt from this fee.
