At Bayless Integrated Healthcare, we are dedicated to assisting our patients with additional resources for a variety of different needs. To the right you will find helpful links, patient forms, information about your rights and responsibilities, as well as insurance & self-pay information. New Patients

If you have never received services by a Bayless provider, please click the button to fill out the attached request form. Upon completion, a Bayless engagement center representative will respond via email or phone call within 24 business hours.

NEW PATIENT INTAKE

Current Patients

If you have an active patient portal, please click the button to self schedule your appointment. If you do not have an active patient portal, please follow the instructions for new patients OR please contact our office at 602-230-7373 for a Bayless engagement center representative to assist you with creating your patient portal for self-scheduling.

DISCLAIMER: If this is an emergency call 911. If you are currently experiencing a crisis please call the crisis hotline at 602-427-4600. If for any reason the patient is unable to keep the scheduled appointment it is the responsibility of the Patient, Parent, and/or Legal Guardian to call Bayless staff, at least twenty-four (24) hours in advance, to cancel. If the Patient fails to cancel their appointment at least twenty-four (24) hours in advance, a $50 no show fee will be applied. This fee must be paid prior to being seen for any Medical, Dietetic, Psychiatric and/or Counseling appointment.  Note: All AHCCCS patients are exempt from this fee.
